FOR IMMEDIATE RELEASE - August 14, 2009 - Yummy Foods Company of San Francisco, California, is recalling its 17 oz./480g Wife Cake because the product contains undeclared egg and wheat. People who have an allergy or severe sensitivity to egg or wheat run the risk of serious or life-threatening allergic reaction if they consume these products.

The product was distributed to individuals in California, Ranch 99 Supermarkets in Kent and Edmonds, Washington and Asian retail markets in California.

No illnesses have been reported.

The products are found in the frozen foods section in a 38 oz. package and were sold at SUPERVALU-owned stores including Acme®, Albertsons®, biggs®, Cub Foods®, Farm Fresh®, Hornbacher’s®, Jewel-Osco®, Lucky®, Shaw’s/Star Market™, Shop ‘n Save®, and Shoppers Food & Pharmacy® in all of the states where they operate.The possibility for contamination was identified through routine sampling of the product at the manufacturing facility. Barr Laboratories, Inc. recalls Dextroamphetamine/Amphetamine 20mg Tablets

FOR IMMEDIATE RELEASE - August 13, 2009 - Barr Laboratories, Inc. is initiating a voluntary recall of Dextroamphetamine Saccharate, Amphetamine Aspartate, Dextroamphetamine Sulfate and Amphetamine Sulfate (Mixed Salts of a Single Entity Amphetamine Product) 20mg Tablets, 100 count bottles, lot number 311756. The product identified is being recalled because the affected lot may contain some tablets exceeding weight requirements which may lead to super-potent tablets.

Potentially clinically significant adverse reactions to a supratherapeutic dose could include cardiovascular, neurologic, psychiatric and gastrointestinal reactions such as: palpitations, tachycardia, hypertension, headache, tremor, tic, dyskinesia, dizziness, blurred vision, sweating, insomnia, agitation, euphoria, mania, anxiety, restlessness, nausea, diarrhea, constipation, dry mouth, and decreased appetite.

This product can be uniquely identified as an oval peach colored tablet, debossed with b/973 on one side and 2/0 on the other side. Barr distributed the affected lot between 06/11/09 and 06/16/09. Only lot 311756 is affected by this recall.

Customers who have this lot in their possession are instructed to cease using the product and return it to their pharmacy/distributor. Wholesalers and retailers should cease distribution and examine their inventory immediately.

FOR IMMEDIATE RELEASE -- August 07, 2009 -- Adams Extract and Spice, LLC. is announcing a voluntary recall of products because they have the potential to be contaminated with Salmonella. The products contain a specific lot of ground red pepper supplied by Van de Vries Spice Corporation, 9 Elkins Road, East Brunswick, New Jersey 08816 This lot of ground red pepper initially tested negative for salmonella. Subsequently, Adams Extract and Spice, LLC. has been informed that another sample drawn from the same lot has been confirmed positive for salmonella.

Salmonella is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis.

The following lots of specific products were distributed in Texas, Louisiana, and New Mexico through retail stores, mail order, internet sales and direct delivery.

The U.S. Consumer Product Safety Commission, CPSC's, Top Ten list of recalled children’s products


WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission (CPSC) today announced its Top Ten list of recalled children’s products as part of the agency’s “Resale Round-up” campaign. CPSC’s goal is to protect consumers by getting dangerous recalled products out of resale stores and off the Internet. Re-selling recalled products is a violation of federal law.

Before re-selling products, CPSC encourages re-sellers to check its Web site, www.cpsc.gov, to determine if the product has been recalled.

“Those who re-sell recalled children’s products are not only breaking the law, they are putting children’s lives at risk,” said CPSC Chairman Inez Tenenbaum. “Resale stores should make safety their business and check for recalled products and hazards to children.”

A CPSC study conducted in 1999 found that nearly 70 percent of resale stores sold at least one recalled or otherwise hazardous product.

CPSC is partnering with the National Association of Resale & Thrift Shops (NARTS), the Salvation Army, state agencies, and others to alert re-sellers about recalled products, product hazards including cribs that do not meet current safety standards, drawstrings in children’s clothing, and the Consumer Product Safety Improvement Act (CPSIA).

“NARTS fully supports CPSC’s outreach efforts to resale stores,” said Adele Meyer, NARTS’ Executive Director. “Our members are resale professionals committed to product safety who believe good value and selling safe products go hand in hand.”

The agency’s education campaign, which will include safety seminars for resale store owners and employees, is part of CPSC’s commitment to protecting consumers, especially children.

CPSC’s Internet surveillance team is monitoring online retailers and auction sites for sales of recalled and hazardous products. Re-sellers are reminded that they must comply with the federal law which will be enforced for in-store and online sales.

If the velcro is not properly re-secured when the flap is adjusted, an infant can slip through the opening and become entrapped between the metal bars and suffocate. CPSC is aware of at least three deaths involving Simplicity bassinets.

Children can fall backwards or fall out of the high chair and suffer bumps and bruises to the head, abrasions, cuts and bruises. Detached hardware also poses a choking hazard to children.

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Safety 1st SmartLight Stair Gates

Units: About 31,500 (100,000 units were previously recalled in February 2009)

Importer: Dorel Juvenile Group USA, of Columbus, Ind.

Hazard: The hinges that hold the stair gate in place can break, posing a fall hazard to children if the gate is placed at the top of the stairs.

Incidents/Injuries: None reported.

Description: The recall involves the Safety 1st SmartLight Stair Gate with model number 42111, which is printed on a sticker under the handle panel. The gates, which were manufactured before December 20, 2008, are white metal with a gray handle and a motion sensor nightlight that illuminates on approach. The manufacture date is located below the gate's handle as a date wheel imprinted in the plastic; an arrow indicates the month, and the year is stamped inside the circle. The manufacture date is also printed on a sticker inside the battery door.

Sold at: Mass merchandise and juvenile product stores nationwide, including Babies R Us, Toys R Us, Wal-Mart.com and other online retailers from January 2005 through July 2009 for about $60.

Name of Product: Jump ‘n Jive™ Doorway Jumpers

Units: About 2,400

Manufacturer: Graco Children’s Products Inc., of Atlanta, Ga.

Hazard: The recalled doorway jumpers include detachable toys that are attached to the jumper straps with strips of hook and loop fabric. The strips of fabric are not permanently attached to the toys and can become detached during use, posing a choking hazard.

Incidents/Injuries: The company has received one report of a child removing the fabric strip and placing it in his/her mouth. No injuries have been reported.

Description: The recalled doorway jumper includes an interactive musical dance mat and two detachable toys. The model number, 1755544, appears on the carton UPC label and on the underside of the jumper tray. The doorway jumpers were sold from April 2009 through July 2009.

Sold at: Babies R Us and other children’s speciality retail stores nationwide and online from Amazon.com for about $55.

Remedy: Consumers should immediately stop using the recalled product and remove and discard the detachable toys and the attaching fabric strips and call the manufacturer for a replacement set of toys. In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Target, of Minneapolis, Minn., is voluntarily recalling about 43,000 Circo Booster Seats. The booster seat restraint buckle can open unexpectedly, allowing a child to fall from the chair and be injured.

Name of Product: Circo Booster Seats

Units: About 43,000

Hazard: The booster seat restraint buckle can open unexpectedly, allowing a child to fall from the chair and be injured.

Incidents/Injuries: Target has received eight reports of the booster seat buckles opening unexpectedly, including three reports of bruises.

Description: This recall involves Circo booster seats with manufacture date codes XJ0811, XJ0812, XJ0901, and XJ0902. The plastic booster seat is blue with green trim and has white straps. The manufacture date code is printed on the backside of the seat next to the consumer warning information. “Circo” can be found on a label located in the front of the seat.

Sold exclusively at: Target stores nationwide from December 2008 through June 2009 for about $13.

Ross StoresTo Pay $500,000 Civil Penalty For Failing To Follow CPSC Issued Drawstring Guidelines

In February 1996, CPSC issued drawstring guidelines (pdf) to help prevent children from strangling or getting entangled in the neck and waist drawstrings in upper outerwear, such as jackets and sweatshirts. In May 2006, CPSC’s Office of Compliance announced (pdf) that children’s upper outerwear with drawstrings at the hood or neck would be regarded as defective and as creating a substantial risk of injury to young children.

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC) announced today that Ross Stores Inc., of Pleasanton, Calif. has agreed to pay a civil penalty of $500,000. The penalty settlement, which has been provisionally accepted by the Commission, resolves CPSC staff allegations that Ross knowingly failed to report to CPSC immediately, as required by federal law, that children’s hooded sweatshirts it sold had drawstrings at the neck.

Children’s upper outerwear with drawstrings, including sweatshirts, pose a strangulation hazard to children which can result in serious injury or death. CPSC and the sweatshirts’ importers announced recalls (07-777, 08-121, 08-146, 08-192) of the products.

In agreeing to the settlement, Ross Stores denies CPSC's allegations that it knowingly violated the law.

Garmin Recalls Garmin® BlueChart g2 and g2 Vision v2009

Out of an abundance of caution, Garmin has voluntarily chosen to globally recall its 2009 version of BlueChart g2 and g2 Vision data cards. In certain waters, there are inaccuracies and/or omissions in the way that this specific version of charting represents depth contours and depth areas, resulting in the depiction of water depth as deeper than the actual depth under certain display conditions

Garmin, Ltd. has announced a voluntary product safety recall of the 2009 version of its marine cartography data card, BlueChart® g2 and g2 Vision. Affected data cards were sold between April 8 and June 3, 2009.

Garmin has received reports of data cards giving inaccurate depth indications in waters along the coast of Sweden and Denmark. As a result, Garmin has chosen to recall all 2009 versions of the BlueChart g2 and g2 Vision cards sold in micro SD/SD card format. Customers are advised not to use these data cards for navigation.

The products subject to recall include:

The ground beef products were produced on various dates ranging from June 5, 2009 through June 23, 2009 and bear the establishment number "EST. 31913" printed on the case code labels. The ground beef products were distributed to retail distribution centers in Arizona, California, Colorado and Utah. Because these products were repackaged into consumer-size packages and sold under different retail brand names, consumers should check with their local retailer to determine whether they may have purchased any of the products subject to recall.

As a result of an ongoing investigation into an outbreak of Salmonella Newport associated with ground beef products, the Colorado Department of Public Health and Environment (CDPHE) notified FSIS of the situation. Epidemiological and traceback investigations conducted by FSIS and CDPHE determined that there is an association between the fresh ground beef products and illnesses reported in Colorado. The illnesses were also linked through the epidemiological investigation by their uncommon pulsed-field gel electrophoresis (PFGE) pattern found in PulseNet, a national network of public health and food regulatory agency laboratories coordinated by the Centers for Disease Control and Prevention.

FSIS would like to remind consumers of the importance of following food safety guidelines when handling and preparing raw meat. Ground beef should be cooked to a safe minimum internal temperature of 160° Fahrenheit.

This particular strain of Salmonella Newport is resistant to many commonly prescribed drugs, which can increase the risk of hospitalization or possible treatment failure in infected individuals.

Consumption of food contaminated with Salmonella can cause salmonellosis, one of the most common bacterial foodborne illnesses. Salmonella infections can be life-threatening, especially to those with weak immune systems, such as infants, the elderly, and persons with HIV infection or undergoing chemotherapy. The most common manifestations of salmonellosis are diarrhea, abdominal cramps, and fever within eight to 72 hours. FOR IMMEDIATE RELEASE -- July 30, 2009 -- United Food Group, Inc announced a voluntary recall of products that contain instant nonfat dry milk manufactured by Plainview Milk Products Cooperative. This recall is a precautionary measure due to the voluntary recall of instant nonfat dry milk announced by Plainview Milk Products Cooperative on June 29, 2009.

Salmonella is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis.

Due to the products potential to be contaminated with Salmonella, the following are being recalled:
