FOR IMMEDIATE RELEASE - April 29, 2010 - Humei Trading Inc of Jamaica, NY is recalling Dried Pachyrhizus, (Product of China), because they contain undeclared sulfites. People who have severe sensitivity to sulfites run the risk of serious or life-threatening reactions if they consume these products.
Dried Pachyrhizus was distributed in the immediate New York City area.
Product is identified in sealed plastic bag (NET WT 350G/12.32 OZ) as HUMEI FOODS DRIED PACHYRHIZUS, PRODUCT OF CHINA.
No illnesses have been reported to date.
The recall was initiated after routine sampling by New York State Department of Agriculture and Markets Food Inspectors and subsequent analysis by the Department’s Food Laboratory personnel revealed the presence of sulfites in the product, which were not declared on the label. The consumption of 10 milligrams of sulfites per serving has been reported to elicit severe reactions in some asthmatics. Anaphylactic shock could occur in certain sulfite sensitive individuals upon ingesting 10 milligrams or more of sulfites. Analysis of the HUMEI FOODS brand DRIED PACHYRHIZUS revealed it contained 139 milligrams per serving.
Consumers who have purchased HUMEI FOODS Brand DRIED PACHYRHIZUS are urged to return it to the place of purchase for a full refund. Consumers with questions may contact the company at (718)995-6818.

FOR IMMEDIATE RELEASE - April 29, 2010 - Northern Food I/E Inc. of Hicksville, NY, is recalling its 100 gram packages of Havista brand "white fungus" because they contain undeclared sulfites. People who have a sensitivity to sulfites run the risk of serious or life-threatening reactions if they consume this product.
The product comes in a 100 gram clear plastic bag with a multi-color label on the front and a white label on the back. The product is coded Best Before 09/10/2011. The recalled Havista brand "white fungus" was distributed in New York and New Jersey in retail stores.

Gap Recalls Baby Swimsuits Due to Strangulation Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Product: Baby swimsuits
Units: About 6,500 in the U.S. and about 480 in Canada
Retailer: Gap Inc., of San Francisco, Calif.
Hazard: The swimsuits have halter straps that were manufactured too short causing the plastic ring located at the neck of the swimsuit to press against the child’s throat and obstruct the airway. This poses a strangulation hazard to the child.
Incidents/Injuries: The firm has received two consumer complaints. No injuries have been reported.
Description: The baby swimsuits were sold in two styles: number 706260 is blue and white and number 700452 is red and white. The style number can be found on the label located on the swimsuit. Both swimsuits were sold in infant sizes up to 24 months and are made of a synthetic knit stretch fabric of polyester and spandex. The straps are made of the same material as the body.
Sold at: The babyGap, GapKids, Gap, Gap Outlet stores nationwide and online at www.gap.com from February 2010 through April 2010 for between $17 and $20.

Hazard: Faulty wiring can cause the connector tips to heat and melt the plastic encasing the connector tips, posing a burn hazard to consumers.
Incidents/Injuries: The firm has received 518 incidents of the connector tips heating, 53 of which resulted in the melting of the plastic casings. Eight consumers have reported a finger tip or hand burn. No reports of medical attention were received.
Description: This recall involves the Targus Universal Wall Power Adapters for Laptops. Only models with the following SKU numbers are affected by this recall: APA23US-02, APA23US-03, APA23US-04, APA63US-03, APA63US-04, APM62US-03 and APM62US-04. The SKU number can be found on the underside of the adapter unit.

Hazard: An “S” hook on the chain connecting the fixture to the ceiling can straighten when subjected to high impact forces, resulting in the light fixture falling. This poses an injury hazard to consumers.
Incidents/Injuries: The firm has received three reports of light fixtures having one chain detached or the fixture falling. No injuries have been reported.
Description: This recall involves the AYC or HBAYC model chain set for use in a gymnasium or similar recreational facility with one of the following Metalux commercial light fixtures: F-Bay HB/2HB; F-Bay 2HBHD; F-Bay HBE; F-Bay HBHT; F-Bay HBI; F-Bay HBL; F-Bay 2HE Series 2HE; Arctic Bay AB; or Arctic Bay ABI.

WASHINGTON, April 29, 2010 - The Murphy House, a Louisburg, N.C., establishment, is recalling 414 lbs. of Brunswick stew products because they contain cracker meal with undeclared allergens, wheat and milk, the U.S. Department of Agriculture's Food Safety and Inspection Service (FSIS) announced today. Wheat and milk are known allergens, which are not declared on the label.

The following products are subject to recall:
  • 5 lb. packages of “MURPHY HOUSE BARBECUE OUR OLD FASHIONED BRUNSWICK STEW” produced on April 2, 2010.
  • 24-ounce cups of “MURPHY HOUSE OUR OLD FASHIONED BRUNSWICK STEW” produced on April 2, 2010.

Each package bears the establishment number “EST. 2135” inside the USDA mark of inspection. Cases and individual packages bear the sell-by date of 05/17/10.

The beef stew products were distributed at the wholesale and retail level in North Carolina.

The problem was discovered during a Food Safety Assessment of operations at the establishment by FSIS personnel. FSIS has received no reports of adverse reactions due to consumption of these products. Anyone concerned about an allergic reaction should contact a physician.

FSIS routinely conducts recall effectiveness checks to verify recalling firms notify their customers (including restaurants) of the recall and that steps are taken to make certain that the product is no longer available to consumers.

Graco®-Branded Drop Side Cribs Made by LaJobi Recalled Due to Entrapment and Suffocation Hazards

Name of Product: Graco®-branded drop side cribs made by LaJobi
Units: About 217,000
Manufacturer: LaJobi Inc., of Cranbury, N.J.
Hazard: The drop side hardware can break or fail, allowing the drop side to detach from the crib. When the drop side detaches, a hazardous gap is created between the drop side and the crib mattress in which infants and toddlers can become wedged or entrapped, posing a risk of suffocation and strangulation. In addition, children can fall from the cribs when the drop side detaches or fails to lock.
Incidents/Injuries: CPSC and LaJobi have received a total of 99 reports of drop side incidents, including hardware breakage and drop side detachment. There were two incidents in which children became entrapped in the gap created by the detached drop side. Both children were freed by their caregivers. There were six reports of children falling due to drop side failure, including one report of a mild concussion.
Description: This recall involves LaJobi-manufactured Graco® wood cribs. The full size cribs were sold in cherry, espresso, natural and white finishes. The production date, item number, purchase order number and finish name is printed on a label affixed to the footboard or headboard. "LaJobi" and the crib model name are printed on a product sticker located on the stabilizer bar or bottom rail of the crib. Affected models are listed below.

Simplicity Cribs Recalled by Retailers; Mattress-Support Collapse Can Cause Suffocation and Strangulation

One Infant Death Reported

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC) is announcing the recall of all Simplicity full-size cribs with tubular metal mattress-support frames. This recall includes fixed-side and drop-side cribs. These cribs pose a risk of serious injury or death due to entrapment, strangulation, suffocation and fall hazards to infants and toddlers.
The crib's tubular metal mattress-support frame can bend or detach and cause part of the mattress to collapse, creating a space into which an infant or toddler can roll and become wedged, entrapped or fall out of the crib.
CPSC has received a report of a one-year-old child from North Attleboro, Mass. who suffocated when he became entrapped between the crib mattress and the crib frame in April 2008. CPSC is aware of 13 additional incidents involving the recalled cribs collapsing due to the metal mattress- support frame bending or detaching, including one child entrapment that did not result in injury, and one child who suffered minor cuts to his head when his mattress collapsed and he fell out of the crib.
CPSC staff urges parents and caregivers to stop using these cribs immediately and find an alternative, safe sleeping environment for their baby. Do not attempt to fix these cribs.
Due to the fact that Simplicity and its successor, SFCA Inc., are no longer in business, CPSC has limited information about the number of cribs sold. All Simplicity drop-side cribs have previously been recalled for a hazard involving the drop side. Simplicity drop-side cribs could still be in use by parents or caregivers who are unaware of the recalls or by those who received a repair kit to immobilize the drop side from Simplicity when the firm was still in business. This recall involves ALL Simplicity cribs with tubular metal mattress-support frames regardless of model number.

Some model numbers are followed by letters, indicating the color or finish of the crib. The name "Simplicity Inc." or "Simplicity for Children" appears on a label on the crib's mattress-support frame and/or the crib's end panels. The cribs were manufactured in China.
The recalled cribs were sold at Walmart, Target, Babies R Us and other stores nationwide for between $150 and $300. Consumers should contact the store where the crib was purchased to receive a refund, replacement crib or store credit.

Important Message from CPSC:

CPSC would like to remind parents not to use any crib with missing, broken or loose parts. Make sure to tighten hardware from time to time to keep the crib sturdy. When using a drop-side crib, parents should check to make sure the drop side or any other moving part operates smoothly. Always check all sides and corners of the crib for disengagement. Any disengagement can create a gap, which could fatally entrap a child. In addition, do not try to repair any side of the crib with tape, wire, rope or by other means. Infants and toddlers have died in cribs with makeshift repairs.

MiDAS Foods International Recalls Instant Beef Soup Mix And Instant Beef Stroganoff Sauce Mix Because Of Possible Health Risk

FOR IMMEDIATE RELEASE - April 27, 2010 - MiDAS Foods International of Oak Park, MI is recalling Instant Beef Soup dry mix and Instant Beef Stroganoff sauce mix because it has the potential to be contaminated with Salmonella, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis.
The dry soup mix and dry sauce mix were not distributed for retail sale to consumers. This notice is to make consumers who may inadvertently come across a package of this product aware of the recall. The products were distributed to food service establishments in the states of Florida and Tennessee.
The dry Instant Beef Soup Mix is packaged in brown cardboard cases dimension: 15"x 7.75"x 5.7" with a brown label titled "Instant Beef Soup Mix" and bearing "Code: AS493060"; Mfg. Date: MAR 10, 2010-01 and produced by MiDAS Foods International / American Saucery. The individual units inside the box are 7" x 6.5" white plastic sealed pouches with plain black lettering "Instant Beef Soup" at the top, and the product code "AS493060" in the bottom right of the text.
The dry beef stroganoff mix is packaged in brown cardboard cases dimension: 15"x 7.75"x 5.75" with a white label titled "Instant Beef Stroganoff Sauce" and bearing "Code: AS605010" in the lower right corner.; Mfg. Date: JAN 21, 2010-01 and produced by MiDAS Foods International / American Saucery. There is a single, unlabled clear plastic bag containing the mix inside the box.
No illnesses have been reported to date.
The recall is the result of notification by one of our vendors that a flavor used in the soup mix contained an HVP (hydrolyzed vegetable protein) produced by Basic Food Flavors of Nevada which had the potential to contain salmonella. Although the flavor used in the soup was tested by an independent lab, and found to be negative for the bacteria, we are proceeding with this notice for precautionary food safety reasons.
Although the soup and sauce mixes are not directly available to the public, if you have obtained a box or package of Instant Beef Soup mix or box of Instant Beef Stroganoff Sauce mix as described above, return it to the place you obtained it. Hazard: The sweatshirts have a drawstring through the hood which can pose a strangulation hazard to children. In February 1996, CPSC issued guidelines (pdf) (which were incorporated into an industry voluntary standard in 1997) to help prevent children from strangling or getting entangled on the neck and waist drawstrings in upper garments, such as jackets and sweatshirts.
Incidents/Injuries: None reported.
Description: This recall involves children's hooded sweatshirts sold in sizes small through extra large. "John Moose" and "Alaska" screen print logos are printed on the front of the yellow sweatshirts. The "Gaxiani" brand name and RN# 55774 are sewn on the neck tag inside of the garment.
Sold exclusively at: Polar Bear Gift Shop in Anchorage, Alaska from February 2008 through August 2009 for about $15.

Remedy: Consumers should immediately remove the drawstring to eliminate the hazard. Consumers can also return the sweatshirt to Polar Bear Gift Shop for a full refund.

The model name and number are displayed on the engine compartment and the serial number is on the right side of the vehicle below the seat on the frame.
Sold at: Ski-Doo dealerships nationwide from July 2008 through February 2010 for between $8,000 and $9,000.

Remedy: Consumers should stop using these snowmobiles immediately and contact their local Ski-Doo snowmobile dealer to schedule a free repair. All known owners of the recalled snowmobiles have been directly notified about this recall by mail.

Jo-Ann Stores Inc., of Hudson, Ohio, to Pay $50,000 Civil Penalty for Violating Federal Lead Paint Ban

Jo-Ann Stores to Pay $50,000 Civil Penalty for Violating Federal Lead Paint Ban (Wed, 28 Apr 2010 15:00:00 GMT) The U.S. Consumer Product Safety Commission (CPSC) announced today that Jo-Ann Stores Inc., of Hudson, Ohio, has agreed to pay a $50,000 civil penalty for allegedly importing and selling children's products that violated the federal lead paint ban.
The penalty settlement (pdf) which has been provisionally accepted by the Commission, resolves CPSC staff allegations that Jo-Ann Stores knowingly imported and sold Robbie Ducky™ children’s products that contained lead paint in excess of the federal government’s regulatory limit from January through November 2007. At the time of the violations, the federal limit on lead in paint was 600 ppm (0.06 percent).
As a result of the Consumer Product Safety Improvement Act of 2008, the regulatory limit on lead in paint and surface coatings was reduced in August 2009 from 600 ppm (0.06 percent) to 90 ppm (.009 percent).
The following Robbie Ducky™ children’s products allegedly violated the federal lead paint ban and were later recalled by Jo-Ann Stores:
  • Robbie Ducky™ children’s watering cans – 6,000 recalled on August 28, 2007
  • Robbie Ducky™ children’s toy rakes – 16,000 recalled on September 26, 2007
  • Robbie Ducky™ children’s toy rakes – An additional 97,000 toy rakes were recalled on October 25, 2007
  • Robbie Ducky™ holiday water globes – 60 recalled on December 13, 2007
In agreeing to the settlement, Jo-Ann Stores denies CPSC's allegations that it violated federal law.
