Hazard: The sweatshirts have a drawstring through the hood that can pose a strangulation hazard to children. In February 1996, CPSC issued guidelines (pdf) to help prevent children from strangling or getting entangled on the neck and waist drawstrings in upper garments, such as jackets and sweatshirts.

Description: This recall involves three styles of children hooded sweatshirts with drawstrings. The zippered sweatshirts were sold in sizes XS through 3XL. The HAY style sweatshirts were sold in black, navy, light grey, charcoal grey and pink. The HKK style sweatshirts were sold in black, navy, light blue, light grey, pink and red. The JMK sweatshirts are reversible microfiber with polar fleece lining and a removable hood with elastic or retractable drawstrings. Style JMK was sold in black only. The sweatshirts have sewn-in neck tag that reads “Gogo Sports” and city names printed across the front of the garment.
Sold at: Clothing retail stores in California, Colorado, Hawaii and New Mexico from January 2008 through December 2009 for between $14 and $27.

Description: This recall involves the Nordic Track Revitalize Inversion Bench 2.0 model 831.14895.0, Gold’s Gym Inversion System models GGBE0867.0 and GGCCBE0867.0 and Weider Club Inversion System model WEBE0878.0. The model number is located under the backrest of the bench. The inversion systems consist of a frame with a backrest, headrest and leg clamp assembly.
Sold at: Sporting good stores, Walmart, Sears and other retailers nationwide from April 2008 through February 2009 for between $200 and $300.

FOR IMMEDIATE RELEASE - April 23, 2010 – BOULDER, CO - Covidien (NYSE: COV) has initiated a voluntary recall of certain lots of its cuffed Shiley^TM tracheostomy tubes and Custom/Specialty tracheostomy tubes due to the product’s cuff not holding air as a result of leaks in the pilot balloon inflation assembly. With respect to the affected units, if a cuff does not hold air, ventilation will be adversely affected since the ability to generate positive pressure in the airway could be compromised by lack of cuff seal. This could result in a sudden decrease in the amount of oxygen in the blood or a sudden increase in the amount of carbon dioxide in the blood, especially if the patient requires assisted mechanical ventilation. In some instances, this could result in serious injury, including death.

The voluntary recall only affects lot numbers from a specific period of time. Only Shiley tracheostomy tubes from lot numbers listed below are affected by this action. Customers should review product in inventory and current use to identify product from the affected lot codes. Detailed steps are provided in the Customer Notification letter for return and disposition of affected product.

The company believes it has taken appropriate steps to address this issue. Adequate supplies of unaffected replacement product are available.

WASHINGTON, April 21, 2010 - Beltex Corporation, a Fort Worth, Texas, establishment, is recalling approximately 135,500 pounds of beef trim products that may be contaminated with E. coli O157:H7, the U.S. Department of Agriculture's Food Safety and Inspection Service (FSIS) announced today.

Each box bears the establishment number "EST. 07041B" inside the USDA mark of inspection on a label. The products were produced on Oct. 28, 2009, Nov. 20, 2009, Feb. 19, 2010, or April 2, 2010, and were distributed to wholesalers and other federal establishments in the States of Georgia, Louisiana, Ohio, Oklahoma, Texas, Washington and Wisconsin.

The problem was discovered by FSIS during a routine Food Safety Assessment performed at the establishment. The establishment's methods for analyzing samples for E. coli O157:H7 in beef products raised concerns about the safety of the product.

FSIS has received no reports of illnesses associated with consumption of these recalled products. Individuals concerned about an illness should contact a physician.

E. coli O157:H7 is a potentially deadly bacterium that can cause bloody diarrhea, dehydration, and in the most severe cases, kidney failure. The very young, seniors and persons with weak immune systems are the most susceptible to foodborne illness.

FSIS routinely conducts recall effectiveness checks to verify recalling firms notify their customers (including restaurants) of the recall and that steps are taken to make certain that the product is no longer available to consumers.

FSIS advises all consumers to safely prepare their raw meat products, including fresh and frozen, and only consume ground beef or ground beef patties that have been cooked to a temperature of 160° F. The only way to be sure ground beef is cooked to a high enough temperature to kill harmful bacteria is to use a food thermometer to measure the internal temperature.
